Obtaining Copies of Your Medical Records

Your medical record at Evangelical Community Hospital contains records of treatment and results of studies performed during your inpatient admission, surgery in our one-day surgery unit, or emergency department visit. Outpatient diagnostic studies ordered by your physician and performed at Evangelical Community Hospital are also a part of your medical record.

You may obtain a copy of the information contained in your medical record or authorize release of the information to a third party. An authorization for release of information must be completed and signed by you or your legal representative prior to release of the information. Birth Certificates

Copies of your official birth certificate cannot be provided by the hospital. You can request a copy.
