From the moment of a cancer diagnosis, you are considered a survivor. Evangelical’s survivorship program provides a range of support to help survivors manage their unique needs and challenges during and after cancer treatment.

    • When calling be ready to provide information such as date of birth, date of appointment, time of appointment, duration of appointment, diagnosis information, type of insurance, and basic contact information.
  • Patients must be traveling to a cancer-related medical appointment.
  • Patient must be able to get in and out of the vehicle unassisted or caregiver assisted.
  • Interpreter services available.

It can take several business days to coordinate your ride. Please call well in advance of your appointment date.
