Home Health
We provide health care in the comfort of your home.

The home environment, away from the sights and sounds of a hospital, can often brighten the spirit and speed recovery. Thanks to today's advanced technology, medical care in the home is possible. Evangelical Home Health, an extension of Evangelical Community Hospital, brings professional health care services that extend from birth through every phase of life into the home.

Services are available to residents throughout the Central Susquehanna Valley. Any resident in the service area may receive care and there is no need to have been a prior patient at Evangelical to access our services.

To be eligible for skilled home health services, you must be under the care of a physician and require skilled care for a medical need that can be met at home on a periodic basis. Referrals may come from you, your family, your physician, a hospital social worker or other agencies. Additionally, terminally ill patients who are no longer receiving curative treatments may be eligible for care through our Evangelical Hospice.

Most home health services are covered by Medicare, Medicaid and other private or managed care health plans. Benefits and eligibility are determined by individual insurance plans and may include a homebound requirement. Financial assistance may be available for patients requiring medically necessary services.

SKILLED SERVICES:

Nursing Services:

  • skilled assessment and observation
  • wound care
  • injections
  • colostomy, catheter, tracheotomy and feeding-tube care
  • blood drawing
  • care of and instruction in intravenous therapy including venous access devices
  • management of diabetes
  • nutrition assessment and education
  • cancer care
  • cardiac care
  • pediatric care
  • prenatal care including high-risk pregnancies
  • childbirth education
  • newborn and postpartum

Home Health Aide Care:

  • personal care
  • exercises
  • light housekeeping
  • laundry
  • meal preparation

Physical Therapy:
Physical therapists evaluate and instruct in therapeutic exercise, transfers and gait training, including prosthetic devices and walkers.

Occupational Therapy:
Occupational therapists evaluate abilities and instruct in activities of daily living, such as bathing, dressing and preparing meals, as well as other activities to help the patient be as independent as possible.

Speech Therapy:
Speech therapists evaluate and treat speech and/or language deficiencies and swallowing disorders.

Medical Social Services:
Medical social workers provide assessment of psychological and social needs, long-term planning, assistance with using community resources, financial planning, and short-term counseling.

In addition, other hospital professionals such as dietitians, stomal therapists and pharmacists are available for consultation and to meet your needs. Because we understand that health care needs can arise at any time, a registered nurse is on call for Evangelical Home Health patients 24-hours a day, including weekends and holidays.

OTHER SERVICES:

Short Procedures
The Short Procedure Program is for patients who do not meet the eligibility for skilled home health services. Short Procedures are designed for the convenience of the patient. In this case, the Evangelical Home Health nurse makes a brief visit to a patient's home to do a simple procedure such as drawing blood. These services are generally not covered by insurance.

Continue Care Services
Our Continue Care program can provide attendants to help you with daily activities to increase your quality of living. Whether you need companionship, light housekeeping and meal preparation, or personal care assistance, this program can help make life easier. Services can be provided 24-hours a day. Fees for Continue Care are generally not covered by insurance, although you may be eligible for financial assistance through programs of the Area Agency on Aging.

In the Past Year...
Evangelical Home Health performed approximately 15,000 home health visits by nurses, therapists, social workers and aides, and our attendants performed about 44,000 Continue Care visits.

Common Questions:
Q: What is covered by my insurance?
A: It depends on the specific plan. Most insurances (except Medicare) require a call for pre-approval. Many have a deductible or a copay.

Q: What does it cost?
A: It depends on the service. Skilled home health for nursing or therapy generally is covered by insurances, at least in part. Attendant care to help with personal care generally is not covered by insurance. People may be eligible for programs through the Area Agency on Aging.

Q: My father lives alone and I'm concerned that he's not managing as well any more. Do you have anyone who can stop in and check on him or what can you do to help?
A: We would have you talk with one of our nurses to determine whether your father's needs would be considered skilled or not. They probably would not, so we could send a nurse to evaluate him for Continue Care visits from an attendant. Having an attendant 2 to 3 times a week for an hour to help with his shower, dressing and make sure he is taking his medications might be enough to keep him living independently.

Important Note:
Patients have a right to choose which home health agency they get their care from. (There are some exceptions based on insurance contracts.)
