The staff will make recommendations based on the resident's needs for equipment, such as a walker or a cane, wheelchair seating, or bathroom needs to maximize their independence with functional mobility. Prior to discharge, the staff may also perform a home assessment, taking the resident and a family member to the home to evaluate the environment and ensure safety within the home.
