Vaccine Clinics
In the event you have previously scheduled a vaccine appointment with Evangelical and get your vaccine at another facility, please cancel your appointment by calling 570-522-4530, option 1 so that dose can be reallocated to someone in need.
Vaccine Distribution and Location
Organizations and individuals will be assigned a date and time for vaccination and informed of the location of the clinic during the registration process.
Is there a charge for the vaccine?
Evangelical will not charge healthcare workers who qualify for Phase 1A of the vaccine distribution. You will be required to provide proof of healthcare employment during the registration process and when arriving for your vaccination.
There will be an administrative fee assessed for individuals not working in healthcare. Health insurance information will be collected for billing purposes during the registration process and at the time of vaccination. Please bring your health insurance card to your appointment.
No one receiving a COVID-19 vaccine will incur any out-of-pocket expenses.
Can someone come with me?
In order to maintain physical distancing requirements and to reduce the number of visitors entering the Hospital, we ask that you arrive for your appointment without a guest. Those registered who have ambulatory issues may be accompanied by one person.
Vaccine Supply
Due to potential shortages in vaccine supply, clinics may be cancelled at any time. You will be contacted as soon as we are aware of a supply issue in an effort to efficiently reschedule you.
What to Expect at the Vaccination Clinic
To help us maintain appropriate physical distance, it's important that you arrive at your scheduled appointment time. Please do not arrive more than 15 minutes early and do not arrive late.
Upon arrival, each person will:
- Sign in, show ID, and provide proof of healthcare employment or insurance information
- Read and sign a consent form
- Receive the first vaccine dose
- Schedule a time to receive the second dose
- Receive a vaccination card which you will need to bring to your appointment for the second dose
- Wait 15 minutes for post-vaccination observation
The appointment will take approximately 30 minutes—15 minutes for the vaccination and 15 minutes for the observation period.
Anyone who arrives at a vaccination clinic without an appointment will not receive the vaccine.
Once You Receive the Vaccine
The COVID-19 vaccine is not a cure for the virus. It’s another tool in our toolbox in our fight against COVID-19. We must continue to practice other mitigation efforts, like:
- Wear a mask when you leave your home;
- Wash your hands regularly;
- Remain 6 feet from others in public; and
- Stay home if you feel ill.
