Finding a primary care provider (PCP) is one of the easiest steps you can take toward a longer, healthier life. The experienced providers at Evangelical Community Hospital are here to be your teammate in health, helping you stay your best self through routine exams and preventive tests. The more your provider gets to know you as a patient, the easier it is for them to spot medical issues early and provide treatment or recommend you to a specialist.

Scheduling with your provider is now easier than ever! Existing Family Medicine and Internal Medicine of Evangelical patients can self-schedule their appointments through the MyChart app. Please note that new patients must become established with a provider before gaining access to the self-schedule feature.

Frequently Asked Questions

How do I Choose a Primary Care Provider?

Check for In-Network Providers
Go online or call your insurance company to find your list of in-network providers. This will ensure your insurance covers as much as possible for your visits and services.

Determine Your Health Needs
Do you have a growing family, or plan on having one? A Family Medicine practice can provide care from ages infant to senior. They are able to treat a wide variety of conditions. If you're more interested in a doctor that's focused on adult care and specialization, Internal Medicine may be a better fit for you!

Consider Logistics
Choose a provider that you believe will work best for your schedule in terms of appointment times, location, and your fluent language. Being able to communicate fully with your provider will ensure you're able to get the best care possible.

Ask Your Office the Following Questions:
• Who will see me if my provider isn’t available?
• Does my provider have experience treating my pre-existing medical conditions?
• What is the cancellation policy?
• How long will it take to get an appointment on average?
• How long do appointments usually last?
• Can I get lab work and x-rays done in the office?

What Does my Provider Care Provider Need to Know?

Family History
Make sure to ask your family members if there are any medical issues that run in your family, from heart issues, diabetes, blood pressure issues, and more! Your PCP will check if any of these medical issues have passed onto you.

Vital Signs, Height, and Weight
Your PCP will check your vitals at each appointment to establish your normal range. This will signal when a potential issue arises that deviates from your average vitals.

Sexual Orientation and Gender Identity
While you may choose not to disclose your sexual orientation or gender identity, any information provided will be kept confidential by law, and our provider will use it to better understand and meet your healthcare needs. Your healthcare team is also able to use your chosen name, regardless of your legal name status. Find more information here

Smoking, Alcohol, and Drug Use
Your provider is a safe space for you to be honest about any chemicals you put in your body. This helps them to be on the look out for potential issues or negative interactions with medication.

Safety at Home
You will be asked a series of questions to determine if you are in a safe environment at home. Your provider can guide you to protective services if necessary.

Mental Health
Your provider will go ask if you have ever felt depressed, contemplated harming yourself, or are otherwise suffering from mental health issues. Being honest with your provider will help them get you the correct diagnosis and treatment to be your best self!
